Article describing a method of preassessment and discharge planning with an emphasis on interdisciplinary teamwork, as part of a comprehensive spinal cord injury (SCI) ventilator-management program. Steps are described, including: the preassessment process; on-site consultation (including follow-up and communication with the patient and family members); the rehabilitation admission evaluation; psychosocial history and assessment; investigation of funding sources; securing durable medical equipment and supplies; coordinating 24-hour personal assistance for home; exploring transportation options; arranging home modifications; discharge education for patients and families; transportation home; and the first 24 hours at home.
